Background and Webinar Overview
5-methoxy-N,N-dimethyltryptamine (5-MeO-DMT) is one of the most interesting stories in the psychedelic renaissance. Limited oral bioavailability, short duration of action, a signature non-dual conscious state, ‘reactivation syndromes’, and reports of mortality with MAOIs all make 5-MeO-DMT a unique and fascinating tryptamine. The popularity of 5-MeO-DMT exploded from 2010-2020 and reports of harm were not uncommon.
I usually try to focus on making evidence summaries or harm reduction guides where I’m able to support my writing. However, with 5-MeO-DMT it seemed that there was just so little written and such a wild west situation that F.I.V.E. and I collaborated to produce a harm reduction manual focused on the pharmacology and drug interaction potential of 5-MeO-DMT. At that time, it was lacking even a single phase I study in humans so the information was an educated guess that extrapolated from case reports and pre-clinical pharmacologic data.
The pace of change has been staggering from the perspective of drug development. Several companies are now developing formulations focused on 5-MeO-DMT. Signaling the serious possibility of approval in the near-term future, one of the drug’s sponsors, Atai Beckley, has been granted a breakthrough designation allowing fast track approval by the Food and Drug Administration (FDA) and was recently proposed to be acquired by the world’s largest pharmaceutical company, Eli Lilly. Moreover, an Executive Order from the United States federal government to increase speed of review and potentially drug approval of psychedelics for medical use has raised prospects of near-term approval further.
While information on combinations with other substances is still sparse with 5-MeO-DMT and the experiments are still in early clinical phases, a few years of experience and the important foundational human trial information available now warrants a serious update. This webinar will briefly cover the pharmacology of 5-MeO-DMT before focusing on providing an overview of the clinical experiments that have been conducted since the guide’s original release. It is also an event that marks the release of an updated version of the F.I.V.E. Guide.
Our formal learning objectives are to:
- Describe experimental data and clinical updates in the development of 5-MeO-DMT
- Highlight key changes to the FIVE Guide between v1 (release May 2022) and v2 (updated July 2026)
- Discuss broader contraindications, medications, or contexts that may raise risks or change responses to 5-MeO-DMT

Dr. Ben Malcolm earned his bachelor’s degree (BS) in pharmacology at the University of California at Santa Barbara, prior to his Masters in Public Health (MPH) and Doctorate of Pharmacy (PharmD) at Touro University California. He then completed post-graduate residencies in Acute Care at Scripps Mercy Hospital and Psychiatric Pharmacy at the University of California at San Diego Health.
After residency he passed his exam to become Board Certified in Psychiatric Pharmacy (BCPP). He began his career as an Assistant Clinical Professor at Western University of Health Sciences (WUHS), College of Pharmacy before transitioning to his current entrepreneurial role as a Psychopharmacology Consultant, Psychedelic Educator, and Founder of https://www.spiritpharmacist.com
Ben envisions a society in which access to psychedelic drugs in a variety of safe and supported settings is available for purposes of psychospiritual well-being, personal development, ceremonial sacraments, and treatment of mental illness. His focus is on the intersection between psychiatric medications and psychedelic therapies. He has given several Continuing Education presentations to pharmacists and other healthcare professionals as well as published over a dozen articles in peer-reviewed literature relating to psychedelics or psychiatric medications.
